June 15, Meerut (Uttar Pradesh): A clinic for Hepatitis C with state of the art point-of-care diagnostic GeneXpert and latest drugs, called direct-acting antivirals (DAAs), was inaugurated today at the PL Sharma District Hospital which is the first of its kind in the country. The clinic is part of the Hepatitis C project being implemented by international, medical humanitarian organisation Médecins Sans Frontiéres/Doctors Without Borders in collaboration with the National Health Mission of Uttar Pradesh.
Hepatitis is an infection of the liver. There are many different types, but types B and C are the most serious, as those can lead to chronic liver disease, liver failure and even cancer. For hepatitis C, there is no vaccine, and the main cause of transmission in India is poor medical practices such as unsafe blood transfusions and use of unsterilized equipment by unqualified medical practitioners, and sharing of needles among injecting drug users.
The project aims to develop a holistic model of care which includes aspects of counselling, health education and also capacity building of local health workers
“This is a one-of-its-kind medical project in the State and I am very happy that we at PL Sharma District Hospital are taking the lead in finding an effective model of care for treating those suffering from hepatitis C,” said Dr PK Bansal, Hospital Director, PL Sharma District Hospital.
Since January, the clinic has screened close to 1,000 patients and has put 356 patients on treatment free of cost. “The aim of this project is to establish an effective and a simplified model of care and to share best practice for replication by State Government,” said Ghada Khemmisi, Field Coordinator, MSF.
Although the latest generations of hepatitis C drugs, direct acting antivirals (DAAs), manufactured in India are available at much lower cost in the country compared to many other countries, it is still out of reach for millions of patients across India and other countries.
MSF is implementing pilot projects in several high-burden countries with the objective of supporting local governments to establish hepatitis C programs and also advocate for reducing the cost of diagnostics and drugs to increase access to care. “As an organisation, MSF strongly believes that not only should treatment be accessible but also affordable for everyone. While we ensure both by providing free care in this clinic, we do believe that affordable access to diagnostics and treatment for all is important and this can only happen through concerted efforts of national governments,” added Ms. Khemmisi.
“There is an urgent need to raise awareness about the spread of the disease through unsafe medical practices. This will help prevent the spread of hepatitis C and reduce the burden of the disease in the State,” added Dr Bansal.

अपनी तरह की पहली हेपेटाइटिस सी परियोजना का मेरठ में उद्घाटन
15 जून, मेरठ: हेपेटाइटिस सी के लिए अत्याधुनिक पॉइंट-ऑफ-केयर डायग्नोस्टिक जेनएक्सपर्ट (GeneXpert)और नवीनतम दवायें, जिन्हें डायरेक्ट-एक्टिंग एंटीवायरल्स (डीएए) कहा जाता है, उपलब्ध कराने वाले एक क्लिनिक का उद्घाटन आज ‘पीएल शर्मा जिला अस्पताल’ में हुआ। देश में अपनी तरह का पहला यह क्लिनिक, उत्तर प्रदेश के राष्ट्रीय स्वास्थ्य मिशन के सहयोग से अंतर्राष्ट्रीय चिकित्सकीय मानवतावादी संगठन ‘मेडिसिंस सैंस फ्रंटियरेस / डॉक्टर्स विदाउट बॉर्डर्स’ द्वारा कार्यान्वित की जाने वाली हेपेटाइटिस सी परियोजना का हिस्सा है।
हेपेटाइटिस लिवर का संक्रमण है। इसके कई अलग-अलग प्रकार होते हैं, लेकिन बी और सी प्रकार सबसे गंभीर होते हैं, क्योंकि ये पुरानी जिगर की बीमारी, लिवर फेल हो जाना और यहां तक कि कैंसर भी पैदा कर सकते हैं। हेपेटाइटिस सी के लिए कोई वैक्सीन नहीं है, और भारत में इसके फैलने के मुख्य कारणों में खराब चिकित्सा पद्धतियां जैसे असुरक्षित रक्त आधान और अयोग्य चिकित्सकों द्वारा उपकरणों का जीवाणुमुक्त किये बिना उपयोग तथा नशीली दवाओं का उपयोग करने वालों द्वारा सुइयों को साझा किया जाना शामिल हैं।
परियोजना का उद्देश्य देखभाल का एक समग्र मॉडल विकसित करना है जिसमें परामर्श, स्वास्थ्य शिक्षा और स्थानीय स्वास्थ्य श्रमिकों की क्षमता निर्माण के पहलू शामिल हैं।
“यह राष्ट्र में अपनी तरह की पहली चिकित्सा परियोजना है और मुझे बहुत खुशी है कि हम पीएल शर्मा जिला अस्पताल में हेपेटाइटिस सी से पीड़ित लोगों के इलाज के लिए देखभाल का एक प्रभावी मॉडल ढूंढने के कार्य का नेतृत्व कर रहे हैं,” डॉ पी. के. बंसल, अस्पताल निदेशक, पीएल शर्मा जिला अस्पताल ने कहा।
जनवरी के बाद से, क्लिनिक ने करीब 1000 मरीजों की जांच की है और 356 मरीजों को नि:शुल्क उपचार पर रखा है। एमएसएफ के फील्ड समन्वयक, गाड़ा खेमीसी ने कहा, “इस परियोजना का उद्देश्य देखभाल का एक प्रभावी और सरल मॉडल स्थापित करना है और राष्ट्र सरकार द्वारा पुनरावृत्ति के लिए सर्वोत्तम पद्धति साझा करना है।”
यद्यपि हेपेटाइटिस सी ड्रग्स की नवीनतम पीढ़ी, डायरेक्ट-एक्टिंग एंटीवायरल्स (डीएए), भारत में निर्मित है और अन्य देशों की तुलना में यहाँ बहुत कम लागत पर उपलब्ध है, लेकिन अभी भी ये भारत और अन्य देशों के लाखों रोगियों की पहुंच से बाहर हैं।
एमएसएफ कई उच्च बोझ वाले देशों में स्थानीय सरकारों को हेपेटाइटिस सी प्रोग्राम शुरू करने में सहायता करने के उद्देश्य से पायलट परियोजनाओं को लागू कर रहा है और देखभाल तक पहुंच बढ़ाने के लिए निदान और दवाओं की लागत को कम करने की दिशा में लगातार कार्यरत है। “एक संगठन के रूप में, एमएसएफ का दृढ़ विश्वास है कि उपचार हर किसी के लिए न केवल सुलभ होना चाहिए, बल्कि सस्ता भी होना चाहिए। हम इस क्लिनिक में मुफ्त देखभाल द्वारा ये दोनों ही प्रदान करते हैं, और हमारा मानना है कि सभी के लिए निदान और उपचार तक किफायती पहुंच महत्वपूर्ण है और यह केवल राष्ट्रीय सरकारों के ठोस प्रयासों के जरिए ही हो सकता है”, खेमीसी ने आगे कहा.
“असुरक्षित चिकित्सा पद्धतियों के माध्यम से बीमारी के प्रसार के बारे में जागरूकता बढ़ाने की तत्काल आवश्यकता है। यह हेपेटाइटिस सी के प्रसार को रोकने में मदद करेगा और राज्य में बीमारी का बोझ कम करेगा,” डॉ. बंसल ने आगे कहा।
